Create forms to collect information in a variety of types, including Text, Choice, Checkbox, File, and more. You can customize the appearance by adding images or videos, applying themes, and more.
Create and edit form channels
Create a channel
Type /form in the blank space or click + on the left side of the block and select Form.
Add a question type
Type / in the blank and select a question type. You can choose short text, long text, email, phone, multiple choice, checkbox, dropdown, or file attachment.
How to set options by question type
Hover your cursor over a survey item and select ... that appears on the right.
You can edit placeholders, change attachment file sizes, enable multiple selections, and set required answers.

Text, image, audio, video files (including heic, heif extensions)
You can add background blocks, images, tables, videos, etc. by typing / in the blank space just like on the page.
Style Modification
Just like with pages, you can change the width, cover, theme, font, etc. by selecting Page Settings and Design on the right side of the screen.
Page break
Type in / where you want to split the page and then select Page Break, and the Next button and page break line will be automatically created. Respondents can click the Next button to view the form page by page.
Set permissions to submit responses
Click the Permissions menu on the right side of the screen and select Submission permissions.
If you want to allow people who are not logged in to Slashpage to submit responses, please select 'Anyone'.
Edit Ending Page
Click on the Form tab and select Edit ending page.
You can add various images, buttons, videos, etc. by typing / in the blank space like a page.
Share forms with QR code
Click the share icon in the top right of the screen, select QR Code , and download it in SVG or PNG format.
Manage form results
Check the full list of responses
Click the automatically added 'Form Result' table view to see all responses by question type. Only available to Administrator members or site owners.
How to add it manually when the form results table is not visible
Older form channels don't automatically have a results table. To add one, select + → Table on the right side of the Form tab, then select Visibility as Only Managers.

Add fields for response management
Click the + on the right side of the table view to add fields such as checkboxes to confirm approval, statuses to show progress, and more.
Check the form summary
Click the Form tab and select Form Summary .
For each menu, you can check the following:

Statistics: Form visits, number of submitted responses, number of bounced users, etc.
•
Form Summary: Aggregate responses by question
•
All Responses: Completed and Uncompleted (In Progress) Responses
See which questions have low response rates among incomplete responses, and modify your questions to increase response rates.
Set form options
Click the Form tab and select Form Settings.
You can apply the following settings:
•
Form is in progress: If deactivated, the survey will end immediately.
•
Start/Close on scheduled date: The survey will start/end at the set time.
•
Maximum number of responses: The survey will end when the entered number of responses is received.
•
Limited to one submission: Each account is limited to 1 response.
•
Redirect URL: When respondents click the submit button, they will be redirected to the URL you entered.
•
Push notifications: Receive a push message when a response is submitted.
